Privacy Notice
This notice explains how Adele Care Limited uses personal data collected through this website, mainly through the pilot application form. It does not cover patient data we process for practices that use Adele Care. Each practice's data processing agreement covers that, and the practice is the controller.
Who We Are
Adele Care Limited is the controller for this data. We are registered in England and Wales, company number 17482582, with our registered office at 21 St. Anns Road South, Heald Green, Cheadle, SK8 3DZ. What The Pilot Form Collects
Your practice name, your name and role, your work email and phone number, your practice management system, the number of surgeries, whether the practice is NHS, private or mixed, and anything you choose to add in the message box. We also record that you ticked the privacy box and when you sent the form.
Please don't include patient information in the message box.
Why We Use It
We use it to decide whether your practice suits the pilot, to contact you about your application and to plan setup if you take part. We won't add you to a marketing list unless you ask us to, and we don't sell personal data.
Lawful Basis
We rely on legitimate interests under Article 6(1)(f) of the UK GDPR. You have asked us to consider your practice for the pilot and we need these details to do that. We use only what the form asks for, and you can object at any time.
Who Else Sees It
The Adele Care team sees it, as do the services that handle it for us: our form provider ([FORM PROVIDER]), our email provider and our web host. They act on our instructions under a contract. If any of them stores data outside the UK, we rely on UK adequacy regulations or the International Data Transfer Agreement to protect it.
How Long We Keep It
If your practice doesn't join the pilot, we delete your application 12 months after we last spoke. If it does, we keep your contact details for as long as the pilot agreement runs and then as that agreement sets out. Our web host keeps standard server logs for security, which are deleted after [LOG RETENTION PERIOD].
Your Rights
You can ask us for a copy of your data, ask us to correct or delete it, ask us to limit how we use it, object to us using it, or ask for it in a format you can take elsewhere. Email hello@adelecare.co.uk. It's free, and we'll reply within one month.
Complaints
Please tell us first so we can put it right. You can also complain to the Information Commissioner's Office at ico.org.uk/make-a-complaint or on 0303 123 1113.
